Grievance Submission Instructions

The grievance form should be used when a concern or issue has not been resolved through direct communication with the appropriate coach, coordinator, or program representative. This process is intended to formally document ongoing or serious concerns related to conduct, policy, or program operations.

The grievance form is not intended for game-related judgment calls, playing time decisions, or officiating concerns. Issues involving immediate safety should be addressed at the time of the incident and reported directly to OLA leadership.

Completed grievance forms must be emailed to the Owatonna Lacrosse Association Executive Committee for review. Grievances will be handled confidentially and reviewed in accordance with Association policies. Please allow up to 7 business days for review and response.
