Coaches Incident Report

The Coach Incident Report should be used by coaches to document incidents involving player safety, conduct, or behavior during practices, games, or team-related activities. This includes situations involving players, parents, spectators, officials, or other coaches that require formal documentation.

This form is intended for timely, factual reporting and helps ensure incidents are reviewed appropriately by OLA leadership. It should be submitted as soon as reasonably possible following the incident. Emergency situations or league-mandated reporting requirements should be addressed immediately and not delayed by form submission.
